Plan together

Difficult decisions become clearer when wishes are discussed early. A family note can help the conversation but is not a legal document.

Talk about what matters

Ask where the person wants to live, who they want nearby and what makes them feel respected. Let the conversation happen at their pace and change over time.

Organise documents

With consent, record where important contacts and documents are kept and who may see them. For authority, inheritance or representation, consult a qualified professional in the relevant country.

Prepare for changes

Set a date to review the plan. For wishes about treatment, palliative care or the end of life, arrange a conversation with the healthcare team. AgeCare does not provide clinical or legal directions for these decisions.
