Returning home after hospital

Leaving hospital is a handover between teams and the place where care continues. Prepare questions during the stay, not only when transport is waiting at the door.

Ask for an understandable summary

Ask what has changed, which medicines to take and which have stopped, when the next appointment is and whom to contact with questions. Ask the team to explain warning signs requiring urgent help for this particular situation. Write down answers and repeat them in your own words so staff can check understanding. Request interpretation or an accessible reading format when needed.

Walk through the first day at home

Can the person enter the home and use the bed and toilet? Who will arrange food, prescribed medicines and agreed assistance? If equipment is needed, ask when it arrives and who explains its use. Do not assume relatives can provide nursing tasks or physical transfers without training. Tell the team what remains unresolved, including nights, weekends and transport to follow-up appointments.

Connect people as well as paperwork

With the person’s agreement, establish who sends the summary to the clinician continuing care and who confirms the appointment. Keep a copy securely. Use home measurements according to the care team’s plan; a Medical Tool Kit result does not replace professional assessment. AgeCare provides guidance and provider links, but does not itself organise hospital discharge or promise a home-care place.
